What are the rules? How is it usually read aloud?

Depends on the system used. There are a few different ways that problem has been dealt with.

The most notible ways I can think of are: classical pronunciation (there's a fancy term for this style, but it escapes me atm) or a modern pronunciation style (which is basically the one I'm familiar with). Most people don't try to pronounce it like reconstructed Classical/Koine Greek pronunciation.

Ok, it's not Modern, it's Erasmian pronunciation that I was thinking of (which is closer to Modern pronunciation than to how the Earliest Christians spoke). I must have mixed up my terminology with Hebrew.

Classical pronunciation is basically reconstructed pronunciation.

(Yes, it's on Ancient Greek, but Koine is basically Ancient Greek lite anyway. :P )
