To "pinch off" means to separate by pinching. So for instance gardeners will often "pinch off" buds from a plant in order to encourage it to grow more compactly.
I'm assuming someone went in with a surgical device of some sort which both severed the group of chaotic cells from the rest of the organism and held them so they could be easily removed.
